Job Description:

Healthcare Outcomes Performance Company is a vertically integrated musculoskeletal outcomes management company. - HOPCo manages physician practices, hospital service lines, population health and value-based care programs, and musculoskeletal delivery networks.
HOPCo is the managing partner of Arizona-based entities, CORE Institute, Northern Arizona Orthopaedics, CORE Institute Specialty Hospital, Michigan-based CORE Institute, and Florida-based Southeast Orthopedic Specialists.
As HOPCo continues to grow, we are looking for a Patient Navigator . -Please see below for the functions and requirements for this position.
ESSENTIAL FUNCTIONS Effectively works with patients, staff, health service providers, and various other external health care agencies to facilitate and manage clinical care activities such as Medical Tourism, specialty scheduling, Anticoagulation Clinic, post-operative follow ups, and medical/injury/security event incident reporting.
Assists patients with barriers to care including but not limited to scheduling appointments and understanding care plans and instructions and helping patients actualize health management plans and goals.
Implement patient care plans that assign the correct patient care setting, correct patient status and/or identify and eliminate barriers to patient flow.
Collaborates with all internal and external customers to establish a process that streamlines the referral of patients to the program for services.
Facilitates the development and deployment of clinical care programs aimed at improving quality and decreasing inappropriate utilization of a population of patients.
Receive requests for assistance and refers to appropriate party for resolution, if the request cannot be resolved within the scope of the position.
Leads in a manner that is patient-centered and respectful of all other customers and stakeholders.
Collaboratively works with stakeholders to develop and implement an ongoing strategic planning process for the clinical care programs.
Completes all daily, weekly, monthly, and annual reporting requirements on time and in the requested format to ensure appropriate documentation of activities for senior leadership.
Provides education and coaching for specific population needs as required by the program or department.
Collaborates with management to implement new, or modify existing, policies and procedures, as needed.
Interviews and records pertinent patient information in the EMR.
Conduct audits and inspections to ensure compliance with established processes, standards, regulatory guidelines, and/or contractual obligations.
Clinic coverage including clinic patient management and surgery scheduling duties as needed.
Other duties as assigned.
Ability to handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
EDUCATION High school diploma/GED or equivalent working knowledge preferred.
Graduation from an accredited Medical Assistant training program at an accredited school or institution and/or a Licensed Practical Nurse (LPN) license.
EXPERIENCE Minimum 5 Years of experience in an orthopedic practice.
Clinical as well as back office experience preferred.
REQUIREMENTS None
KNOWLEDGE Knowledge of the health care field and orthopedics.
Knowledge of policies and procedures with regard to patient care and scheduling.
Knowledge of back office skills to include injections, suture and staple removal, orders, and intake.
Knowledge of patient confidentiality, state laws, HIPAA regulations and medical records procedures
SKILLS Skill in follow-up with patients and responding to inquiries in a timely manner.
Skill and professionalism when interacting with providers, patients, family members and the public.
Skill in understanding patient education needs by effectively sharing information with patients and families.
Skill in problem-solving and time management.
ABILITIES Ability to project a pleasant and professional image as a CORE employee.
Ability to plan, prioritize and complete delegated work assignments.
Ability to demonstrate compassion.

ENVIRONMENTAL WORKING CONDITIONS Normal office environment.
Some travel within community.
PHYSICAL/MENTAL DEMANDS Requires sitting and standing associated with a normal office environment.
Some bending and stretching required.
Manual dexterity using a calculator and computer keyboard.
