Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.Community Bridges is seeking a Patient Care Tech for our multiple locations throughout Arizona. The Patient Care Tech will work as part of the clinical team dedicated to behavioral health and substance use. As Patient Care Tech, you will coordinate services with the goal to provide experience, strength, and hope to individuals, their families, and significant others. Responsibilities

• Obtains vital signs, performs urinedrug screening, obtains blood glucose levels and other medical care duties as delegated by the RN.
• Orient patients to treatment setting and assists with activities of daily living as needed.
• Participates in implementation of the patient plan of care.
• Completes rounds and monitors patient activities, physical and mental status and reports patient status to nursing staff.
• Performs bathroom monitoring, bed checks, and special monitoring as directed by the RN or RN Supervisor.
• Checks patient belongings for contraband to ensure safety for the person while in treatment at CBI.
• Documents observations, interventions, and group activities in the patient medical record while maintaining confidentiality of all health information.
• Assists in maintaining a safe, orderly & therapeutic environment. Requirements
  • Six months of behavioral health work experience
  • Minimum of a high school diploma or GED
  • Arizona Driver's License with 39 months clear MVR
  • Level 1 Fingerprint clearance card.Benefits
