individuals with a valid driver's license to join our team. If
you're ready to get moving in a new role, head to our location at
3146 E Windsor Ave Phoenix, AZ 85008 on Tuesdays and Thursdays
between 12pm and 2pm for an on-the-spot interview!
Direct Support Professional- Day Program (LEAF) FunctionsOur day
program is Monday through Friday.
- Supervision of the Members' health, safety, and security to
include assistance with Activities of Daily Living (eating,
bathing, dressing, toileting, ambulation, and continence).
- Coordinates the instruction and supervision of the Members in
appropriate basic activities of daily living skills and
recreational events in accordance with the Person-Centered
Plan.
- Encourage the Members to develop and maintain positive
interpersonal skills.
- Coordinate transportation, transport, and/or accompany
according to their Person Center Plan.
- Ensure proper medication procedures are followed for the
Members.
- Ensure physicians' orders are implemented correctly.
- Responsible for developing/assisting to develop, implementing,
documenting, and monitoring of each Member's
program.Requirements:Job Requirements:
- Minimum of 18 years of age
- Current driver license with Non-Restricted Motor Vehicle Record
and compliance per the requirements of The Opportunity Tree's
insurance carrier.
- Excellent communication skills, including the ability to write,
speak, and listen effectively in English.
- Experience: Preferred in the Intellectual and Developmental
Disabilities (IDD) field.
- Education: High school diploma or GED
- Knowledge: Beginner MS Outlook, MS Word, MS Excel, web
browsing
- Ability to work collaboratively as a team member with Members,
staff, families, funders and stakeholders.
- Successful completion and maintain all required training which
may include CPR, First Aid, and Prevention and Support.
- Successfully meet AZ-DPS requirements for Fingerprint Clearance
Card.
- Reliable transportation and willingness to drive company
vehicles for The Opportunity Tree business.
- Skills to include but not limited to knowledge of principles in
staff interaction, task productivity, time management, problem
