POSITION SUMMARYThe responsibility of the Patient Care Coordinator (PCC) is to facilitate care between primary care, behavioral health, psychiatry, and case management teams regarding patient health care needs. This individual will record progress and status of internal/external referrals in the patient's chart and is accountable for the information flow of internal/external referrals between patients and the clinical/medical teams.This is a non-exempt position that reports to the Clinic Manager.Essential Functions--- Provide ongoing treatment coordination with behavioral health, primary care, psychiatry, and case management teamso Initiate coaching, information, and support to empower patients to become an active participant in their treatment opportunities and planningo Actively coordinate care and provide regular staffing/updates with the treatment teamso Advocate patient health care needs--- Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patient and their familyo Participate in regular team meetings, departmental, and organizational committees Demonstrate empathy and compassion in all patient interactions--- Create, update and access confidential patient information while adhering to HIPAA guidelineso Verify patient informationo Perform routine data entry into various software systemso Answer or return patient and provider inquirieso Add information to appropriate software resourceo Schedule appointments within specified guidelines--- Other duties as assignedQUALIFICATIONSEducation and Experience Requirements--- HS Diploma (or GED) (Required); Bachelor's in related field (preferred)--- Valid AZ LPN or LVN license--- Valid and current CPR, First Aid Certification, and an Arizona Fingerprint Clearance card--- Ability to communicate effectively orally and in writing, demonstrating professional writing skills--- Excellent communication skills virtually and in person--- Basic to intermediate knowledge of customer service protocols and patient engagement strategies--- Knowledge of, and ability to use, electronic record keeping systems (EMR, billing software, insurance carrier portals, spreadsheets, etc.)--- Bilingual English/Spanish fluency preferredTools and Equipment Requirements--- Up to date with advancements in office gadgets and applications (i.e., phone, computer, printer, copier, etc.)--- Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint.--- The ability to use the internet and various web browser software is required
