Job Description:

Duties and Responsibilities:

Working with clients and their families who have unique behavioral health needs

Working with the geriatric and long term care population

The ability to work flexible hours based on the needs of the clients and families served, which may include evening or weekend hours

Must possess good verbal and written communication skills in order to articulate the client's progress

Write progress notes and other applicable clinical documentation

Attend on-site training and meetings

Have reliable transportation

Follow agency and professional ethics in maintaining client confidentiality

Performs other duties as assigned by the manager

Counseling sessions at least two times per week or as needed

Provide guidance and encourage the development of the client's personal coaching style

Provide written and verbal feedback to assist clients with set goals

Develop and implement successful intervention techniques including identifying the treatment method, establishing treatment plans that identify problems, goals, and measurable objectives that are achievable within a reasonable time frame, interventions/strategies that address the assessment issues, and maintaining timely progress notes that reflect movement towards goals/objectives.

Conduct educational activities to promote client understanding of treatment issues including medication, diagnosis, and parenting skills.

Complete treatment plans and other clinical reports under the supervision of an independently licensed professional.

Coordinate with a multidisciplinary team regarding patient care. Maintain patient confidentiality in the management of information and in accordance with HIPAA rules. -Participate in mental health staff and department meetings. -Adhere and model the code of ethics associated with license or certification. -Meet regulatory, licensure, health assessment, and CPR requirements.

Other duties as assigned.

Requirements:

In-home individual or family counseling

The AC is a mobile position and FACTS Behavioral Health requires its employees to maintain reliable transportation. The AC position is responsible for traveling to and from appointments throughout Maricopa County.

Education requirements
Master's Degree in Sociology, Social Work, Psychology, Counseling or related field of study from an accredited college or university
Experience requirements
Experience working with mental health/behavior health crisis work, in-person counseling, and clinical documentation.
