occupational medicine health services and electronic health records
management for sworn staff from both the City of Phoenix (City)
Fire and Police Departments as well as public safety personnel
assigned to other jurisdictions throughout the region who have
contracted with Phoenix Fire Department Health and Wellness Center
(PFDHWC) for these services. Valor is different because of our

to none. Location Phoenix Fire Department Health and Wellness
Center (PFDHWC) 50 S. 12th St., Phoenix, AZ 85034 PFDHWC Hours
Facility hours are 7:30 a.m. to 4:00 p.m. Monday through Friday.
PFDHWC Clinic hours are 7:30 a.m. to 12:00 p.m. and 12:30 p.m. to
4:00 p.m. Monday through Friday, with a 30-minute lunch period.
Holiday Closures The PFDHWC is closed on all City of Phoenix
observed holidays. Requirements: Responsibilities: Proficient in
resting 12-Lead EKG’s, V02, C-PET and stress tests. Schedule
Paramedic to perform treadmill testing based on PFD supplied
paramedic list in accordance with PFDHWC scheduling guidelines.
Co-Manage with PFD Clinic Operations Manager and the Medical
Director-the PFD’s Tier 4 Health Assessment Program assuring
consistency in follow-up appointments and that required medical
documentation is provided to clinicians upon request. Schedule make
up treadmills utilizing the electronic health record and Telestaff.
Ensure that the scheduled PFD Paramedic checks that the PFDHWC drug
box is locked, inventory is checked and logged daily using
electronic patient care reporting on the PFD supplied iPad. Perform
daily treadmills monitoring patient heart rate reporting any issues
to the provider. Assist with the preparation of charts and the flow
of patients as needed. Perform back-office evaluations, vision,
pulmonary function, hearing, and the InBody as needed. Perform
phlebotomy and administer medications as needed. Enter back-office
evaluations into Electronic Medical Records system. Assist
clinicians with data collection for studies. Assist clinicians with
the treatment of medical and industrial patients. Assist with
maintenance and stocking of medications and medical supply
inventory. Assist with maintaining a clean, functional environment
through regular cleaning of equipment, sterilizing medical
instruments, cleaning exam rooms and storage area; notifies staff
member regarding ordering supplies and assists with stocking
supplies; assists with restocking exam rooms; performs basic
maintenance; notifies support staff regarding any necessary
repairs. Follow manufacturer’s recommendations and OSHA guidelines
in handling hazardous substances. Responsible for the proper
collection of specimens, related paperwork, and delivery of
specimens to the appropriate place. Minimum Qualifications:
Registered Nurse in the State of Arizona. Five years of clinical
experience. Current ACLS certification. Current CAOHC and NIOSH
certifications or acquired within one year of Contractor’s hire
date to work at PFDHWC. Two years of experience and knowledge in
stress treadmill testing. Must be able to use various personal
computer software programs including, but not limited to,
electronic medical records system, Outlook, and Microsoft Suite.
Ability to prioritize and handle a variety of tasks simultaneously,
with frequent interruptions and able to work independently.
Preferred Qualifications: Bachelor of Science in Nursing from an
accredited institution. V02 and/or C-PET testing experience Working
knowledge of and experience with the IAFF/IAFC Wellness Fitness
Initiative, NFPA 1582 and OSHA 1910 relating to firefighters.
Experience with tactical athletes in a first responder environment.
This job is CONTINGENT upon contract award, client approval,
completion of a favorable background investigation, and the ability
to obtain and maintain a Public Trust / High Risk clearance. Final
compensation will be based on relevant factors including work
experience, skills, certifications, and internal equity. Qualified
