Registered Nurse (RN) | Rehab Location: Phoenix, AZ Agency: Mindlance Health Pay: $1,979 per week Shift Information: Nights - 3 days x 12 hours Contract Duration: 13 Weeks Start Date: 1/26/2026 About the Position TravelNurseSource is working with Mindlance Health to find a qualified Rehab RN in Phoenix, Arizona, 85001! ID: 74943882 Shift: 12H Nights 12hr nights, 4 total weekend shifts per 4 weeks, holidays based on unit coverage Description: Unit Notes:MUST HAVE LEVEL 1 TRAUMA EXP 50 mile radius rule, DL is source of truth, please include with submissionMedical Solutions Pre-Screens, if clinical recommends, sends to manager for approval to offer, no 2nd interviewmust pass dysrhythmia exam prior to acceptance, traveler has 2 attempts., if failed, will be DNU with CS AZ and Sac Region for 6 months*Travel Teams must have their own vehicles*must have natural hair color and no facial piercings*men must clean shave for fit test*pass fail exam on day 1 of orientation - details to followDAY 1 PASS FAIL SIMs Lab: All RNs will be expected to complete SIMs Lab for Foley catheter insertion & central line dressing changes, with the exception of NyICU & ED. NyICU only has to do Foley catheter insertion. ED does Foley catheter insertion but does port-a-cath insertion instead of central line dressing change. This is for travelers going to St. Joseph main campus &/or St. Joseph Westgate campus. NOTE - SIMs lab is pass/fail & nurses have been termed during SIMs Lab for breaking sterile field. Those travelers that understand sterile field typically pass SIMs Lab skills (See separate notes for the specific checklist IF traveler asks for additional details). The hospital expects that every nurse is competent in these areas, especially maintaining sterile field, as these are nursing skills that are performed frequently. NOTE: if a candidate indicates that they are not competent in either of these areas, they are probably not a good fit. In addition to SIMs Lab, they will also go through lifts, restraints, & wound care, but these are just educational based since the equipment & policies can be different facility to facility. - Unit: Neuro Rehab- beds: 52- Certs: RN BLS, FIM / Tech BLS, FIM. RN with strong MedSurg experience required, Rehab experience preferred.- Ratios: RN 1:6, Tech 1:8- EMR: Cerner- Charge Nurse: 2 Charge RNs with a light assignment depending on census.- Nurse Aides: Yes- Weekend Requirement: 4 weekend shifts in 8 week period.- Holidays: Work into core rotation.- Scrub Color: Slate grey.- Tele: No- Vents: No- Hospitalist or Intensivist in house: 3 Rehab Physicians round daily. On-call at night.- IV Team: SWAT RN available for difficult IV sticks.- RT 24/7: Yes- Pharmacy 24/7: Yes- Shifts: \"per job positingTechs - Start times 630a, 3p, 1130p\"- Floating: Yes, any MedSurg unit.- Floor specific orientation: 2 shifts.- Common diagnosis / Types of patients: 90% neuro population, low stimulation area, TBI, brain tumor resection, CVA, lung transplant, liver transplant. - Additional Notes: Busy times 6-9a, 630-9p. Interdisciplinary team rounds. 5/7 days, 3hrs per day therapy. Average LOS 19-21 days.- Additional Questions: Nurses need to be comfortable with high level of communication and personal involvement with patients and family. Partnership of care.
