Will Do - Essential Functions The Manager of Care Delivery plays a
pivotal role within Emcara Health, championing the seamless
integration of clinical services and operational excellence to
ensure patients receive the highest standard of care. This
leadership position is responsible for overseeing multidisciplinary
teams, optimizing care processes, and advancing organizational
goals through evidence-based management, quality improvement
initiatives, and compassionate leadership. Team Leadership and
Development Mentor and lead a high-performing team, managing
day-to-day activities including case assignments, performance
management, and workload balancing. Foster a culture that embraces
change, collaboration, accountability, and professional growth.
Operational and Clinical Oversight Ensure clinical operations
support high-quality patient care, including team assignments and
workload balance. Collaborate with leadership and medical directors
on patient care and team performance. Collaboration and
Relationship Management Build and maintain collaborative
relationships with physicians, allied health professionals, support
staff, and external partners. Represent the care delivery team in
organizational committees, task forces, meetings and as needed for
community events. Who You Are Required Qualifications Bachelor’s
degree or higher in Nursing, Healthcare Administration, or a
related field Current professional licensure as a Nurse
Practitioner 5 years of progressive clinical experience 2 years in
a supervisory or management role preferred. Demonstrated expertise
in care coordination, patient safety, and quality improvement.
Strong knowledge of healthcare regulations, accreditation
standards, and best practices in care delivery. Strong leadership,
organizational, and interpersonal skills. High proficiency in
electronic health records (EHR) and data-driven management tools.
Ability to pass background check upon hire and throughout
employment to include criminal felony & misdemeanor search, SSN
validation/trace search (LEIE), education report (highest degree
obtained), civil upper and lower search, 7-year employment report,
federal criminal search, statewide criminal search, widescreen plus
national criminal search, health care sanctions-state med (SAM),
national sex offender registry, prohibited parties (OFAC)
(terrorist watchlist), and a 10-Panel Drug Screen. Valid
unencumbered state/region driver’s license Valid proof of
sufficient car insurance coverage for the state/region appointments
are scheduled to be completed in. Ability to efficiently provide
the credentialing department with all documentation and
requirements to obtain and maintain the appropriate licensure and
certifications to practice within the state/region of residence.

Conditions: Ability to lift 20 lbs. Ability to bend or kneel for
extended periods of time if needed. Additional physical demands
include constant ability to remain in a stationary position, move
about inside and outside of patient homes, able to communicate and
exchange information with others, able to inspect information, able
to perform repetitive motion with arms and fingers, and drive to
and from patient homes. Mental demands include constant ability to
interpret data, problem solve, make decisions, and organize and
plan. Work may be performed in patient homes and potentially
exposed to adverse environmental conditions. This position may
require travel within the assigned market and may require
occasional evening, weekend, or on-call hours depending on
operational needs. Frequent interaction with patients, families,
staff, and external agencies. May involve exposure to stressful
situations and the need for rapid problem-solving. We encourage
