  • Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission.
  • Banner has a specific cover page!
  • Vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in.
  • This helps us get candidates presented quicker as we all know these orders open/close quick!
  • This form needs to be 100% accurate and have all request noted in the highlights section.
  • Start Date should be on or very quickly after the Order Start Date listed.
  • All RTO should be listed on the cover page, must be less than 7 days.
  • RTO will not be approved in the first 2 weeks of assignment.

    • Job Requirements
    • Minimum 2 years of current PCU experience - REQUIRED
    • Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
    • No Nursys Hits - REQUIRED
    • Travel experience - Preferred
      • Schedule Information
      • All shifts the traveler is open to working should be listed on the submission.
      • Banner will not approve RTO within the first week or holidays.
      • RTO can not exceed 7 days.
