Job Description:

A health services network in Arizona is currently seeking a licensed Physician, with an Internal Medicine background, to join their Primary Care team at one of their practices in Phoenix.Responsibilities:

The Primary Care Physician will:

• Provide best in class comprehensive primary care in an outpatient setting
• Partner with the Care team and leverage the network's coaching and integrated behavioral health models to engage the geriatric patient population in care and drive behavior change
• Utilize the proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population
• Perform other duties, as neededQualifications:
  • 2+ years of Primary Care experience
  • Licensed to practice Medicine in the State of Phoenix
