Assists, observes, and reports patient changes to licensed caregiver and maintains accurate medical record documentation relative to the care delivered.Provides and collaborates direct patient care services under the direct supervisor of a licensed caregiver. Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, assists/leads in delivering a wide variety of social and recreational activities and implementation of selected portions of the patient care plan as directed under supervising nurse.Assists, observes and reports changes in patient condition to licensed caregiver and maintains accurate medical record documentation in relation to the care delivered.Addresses and/or directs questions or concerns from patients, families, caregivers, or health care team members.Follows proper CACFP and organizational procedures; organizes data; completion of reimbursement claim forms.Performs other duties as assignedFacility: Desert Mission Department: DM Adult Day Hlth Care Work Hours: 8:00am-4:00pm Shift: 01 - Days Position Type: Regular Full-Time
