A very reputable Federally Qualified Health Center near Phoenix, AZ is seeking a 3 to 6-month Locum Tenens Physician for their Family Practice position.
Pay Rate: $150.00-$160.00 Per Hour (All-inclusive, 1099 OR W2)
Locum Tenens Physician Job Details:

• 
  
• The family-practice or family-medicine physician must have a current, unrestricted California medical license
  
• Respite Care: Inpatient
  
• Clinic Care: Outpatient
  
• 1099 or W2
  
• Weekly direct deposit!
  
• Opportunity to go permanent after Locum Tenens Physicians contract or extend contract

• Average patients seen per day: 18
  
• Setting: Federally Qualified Health Center
  
• Schedule: Monday - Friday, 8:30 AM to 5:30 PM
  
  Family-Medicine Physician Qualifications:
  
  • 
    
  • Doctors must be graduates from an accredited MD or DO program.
    
  • Must be board-certified in family medicine or internal medicine; If internal medicine must have previous primary-care or urgent care experience.
    
  • Fully vaccinated for COVID-19 with booster by 2/1/2022 is a federal requirement for this position
    
  • 1-year of post-residency experience is required
