Family-Centered care that focuses on the needs of the child first and values the family as an important member of the care teamExcellence in clinical care, service and communicationCollaborative within our institution and with others who share our mission and goalsLeadership that sets the standard for pediatric health care today and innovations of the future Job Summary: The Director of Clinical Integration works in coordination with and reports to the Chief Medical Officer to define and implement PCCNs quality improvement program and care management strategies. Primary Duties I. Works with the PCCN Quality Committee to define the CI programs quality agenda. II. Monitors quality performance indicators for the local population of PCCN patients. III. Defines, develops and implements care management approach for PCCN populations. Interactions include Home Care, departmentally-based Disease management resources, and office-based care coordinators and or clinicians, appropriate city, county or other government agencies. IV. Contributes to development and implementation of standardized clinical care processes across PCCN membership to streamline care. V. Communicate with targeted PCCN patients on behalf of member physicians to: facilitate patient understanding of conditions, medications, and healthinformation resources. Coordinates appropriate testing, Coordinates appropriate preventive care and disease care. Directs patient to appropriate resources for followup. Interacts with Clinical Integration Committee to track, review, and approvemetrics. VI. Works with contracting team to ensure appropriate quality targets and care models are developed with payers. VII. Uses relevant data analytical tools to manage and define quality and care model targets. VIII. Performs miscellaneous job related duties as requested. Qualifications: Education Requirements:Masters degree in Project Management or related field - preferred Experience Requirements:Experience in clinical quality programs and metrics in health care setting Experience in outpatient and inpatient clinical processes. Certification/License/Registry Requirements:Care Management certification - Preferred Special Skills & Requirements 1. Excellent communication skills, both oral and written. Analytical Thinking - Has the ability to address complex situations and problems by breaking them into smaller pieces, organizing and comparing them systematically and identifying causal relationships. Appears methodical and calculated in his or her approach to problem solving. Motivational Skills - Is capable of "rallying" people to a common goal. Has the ability to manage the emotions of others and focus their energy to achieve a difficult but desired outcome. Communication Skills - Speaks with precision. Prefers straight talk and is adept at cutting through to the heart of the matter. Is an active listener, attentive to the other person's message, and provides verbal and non-verbal cues signaling his or her attentiveness and comprehension of what is being said. Is able to present and summarize a topic clearly and succinctly. Interpersonal Skills - Has the ability to hear accurately and understand unspoken or partially expressed thoughts, feelings and concerns and react appropriately. Is open, non-defensive, and easily gains rapport. Appears to have a "sixth" sense.
