That means you'll have an opportunity to make an impact on a huge scale - as part of an incredible team culture that's defining the future of behavioral health care.The operating hours for this team Monday - Friday: 8:00am to 5:00pm in the employee's time zone. - (There is flexibility to work later, if needed, but not earlier)You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. -Primary Responsibilities: -

  • Provide case management to families as they navigate the complex maze of social service, medical, and education systems to get special needs care for their children
  • Act as a single point of contact to serve the entire family's needs behavioral health needs
  • Identify high-risk, co-morbid needs of consumers
  • Link the consumers and their families/caregivers with connections to appropriate pediatric and family referrals and services
  • Work with members and their family via telephone to address healthcare needs
  • Provide supportive follow-up, monitoring and education as indicated
  • Explain behavioral health insurance benefits and what behavioral health services are available
  • Accept referrals via designated processes, collaborate in evaluating available services, and coordinate necessary psychiatric and community referrals, as needed
  • Partner with internal Optum Behavioral Health Services Teams to coordinate a seamless transition for consumers from an inpatient setting to community-based service
  • Collaborate with providers to determine acuity of mental health concerns, barriers and progress; coordinate appropriate services
  • Partner with designated external providers, vendors, programs, entities to address the needs, gaps in care -
  • Establish and foster positive relationships with medical partner to offer a seamless, holistic case management product to member
  • Generally, work is self-directed and not prescribed
  • Works with less structured, more complex issues

Required Qualifications: -
    • Master's Degree in Psychology, Social Work, Counseling or Marriage or Family Counseling; or Licensed Ph.D. or Registered Nurse
    • Independent, current, unrestricted independent license to practice in the state of residence (i.e. LP, LPC, LCSW, LMHC, LMFT or state equivalent) -
    • 2+ years of experience in behavioral health
    • 1+ year of recent experience working with children/adolescents/families
    • Proven solid computer skills at the intermediate level, proficiency with MS Office to include Teams, Excel
    • Access to high-speed internet from home (Broadband Cable, DSL, Fiber) and a dedicated workspace from home
    • Ability to work within business operating hours of Monday - Friday: 8:00am to 5:00pm (or later) in your time zonePreferred Qualifications: - - - - - - - - -
      • Experience in case management, inpatient coordination and transition of care -
      • Experience creating care plans and identifying resources for children/adolescents and their families
      • Experience with the pediatric population
      • Experience with families of children diagnosed with Autism
      • Experience in dual diagnosis with mental health and substance abuse
      • Experience in a community mental health setting
      • Experience in a managed care environment
      • Experience working in an environment that required coordinating benefits and utilizing multiple groups and resources for patients -*All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy - -California, Colorado, Connecticut, Nevada, New Jersey, New York, Rhode Island, or Washington Residents Only: The salary range for California, Colorado, Connecticut, Nevada, New Jersey, New York, Rhode Island or Washington residents is $56,300 to $110,400 per year.
