Job description:The Case Manager is responsible in providing recovery-oriented rehabilitation services in group or individual settings. Case Manager duties include assessing, planning, implementing. monitoring and evaluating actions required to meet the client's health and human services needed.Responsibilities:

• Create ISP's, Discharge Plans, Annual Assessment for all new residents
• Schedule and participate in monthly ART meetings with clinical and outpatient teams
• Complete monthly CCR to identify resident care need and to justify stay
• Implement and review treatment plans as needed
• Collaborate with additional treatment providers and placement agencies
• Ensure all documentation is completed in a timely manner
• Coordinate and accompany residents to provider appointments
• • Conducting comprehensive assessments of patients' needs and developing individualized care plans.
  • Coordinating care across multiple providers and settings, including hospitals, primary care providers, and community-based resources.
  • Monitoring patients' progress towards treatment goals and making adjustments to care plans as necessary.
  • Advocating for residents to ensure that they receive the best possible care and services.
  • Providing education and support to patients and their families to help them manage their health and well-being.Qualifications:
    • A High School Diploma/GED and three years of relevant experience in a behavioral health setting or an
    • An Associates and or bachelor's degree and two years of relevant experience in a behavioral health setting
    • Must be at least 21 years of age.
    • Minimum typing speed of 25 wpm.
    • Some assignments require an Arizona Fingerprint Clearance Card
    • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR) PI207922490
