Overview
Terros Health is pleased to share an exciting and challenging
opportunity for a Children's Wellness Coach position, based out of
locations in Phoenix, AZ, Tempe, AZ, and Chandler, AZ. This is a
direct service position working in collaboration with the Boys and
Girls Clubs of the Valley at assigned sites. This position will
interact directly with the Children and Staff and Families at the
Clubs and use the designated prevention model Positive Action while
also implementing other clinical approaches based on Club needs.
This team member needs to have a passion for working with Children
in an afterschool setting, be willing to be an active
participant/lead in therapeutic activities, and act as a liaison
for Terros Health and these co-locations. Terros Health is a

Seeking a Children's Wellness Coach for a brand new, exciting
collaboration with the Boys and Girls Clubs of the Valley at
assigned BGCAZ locations. This position will interact directly with
the Children and Staff and Families at the Clubs and use the
designated prevention model Positive Action while also implementing
other clinical approaches based on Club needs. Full-Time Employed:
40 hours/week M-F: 11a-7p Starting at $19/hr Location: co-located
within ONE Boys and Girls Clubs of the Valley location (listed
below): Compadres Branch - 300 E. Chandler Blvd, Chandler, AZ 85225
I.G. Homes Branch - 1601 W. Sherman St, Phoenix, AZ 85007
Rosenzweig Branch - 2242 W. Missouri Ave, Phoenix, AZ 85015 North
Tempe Branch - 1555 N. Bridalwreath St, Tempe, AZ 85288 Bilingual
in Spanish is a plus (Additional Language Differential Pay

excellent development opportunity Duties include:
- Conducts the engagement process with
families to identify natural/formal supports as outlined in the
Arizona Model, Child and Family Team (CFT) process, and Regional
Behavioral Health Authority (RBHA) for enrolled families.
- Facilitates CFT/Service Plan meetings according to the Arizona
Model and RBHA guidelines in order to create individualized service
plans as identified by the CFT members.
- Ensures families understand the behavioral health process, the
role of CFT Facilitator, and ensure family participation that links
them to natural or community-based supports.
- Coordinates and schedules CFT/Service Plan meetings with
natural, formal supports, and system stakeholders
- Submits appropriate referrals to clinical outpatient services
within agency and other behavioral health providers identified by
the CFT.
- Updates and maintains electronic medical records on each case
which include but not limited to; Annual Assessment Update, RBHA
demographics, Child and adolescent service intensity instrument
(CALOCUS), and CFT/Service Plan
- Completes strength assessments and crisis/safety plans with
families according to RBHA and the Arizona Department of Health
Services (ADHS) guidelines.
- Develops positive supportive relationships with consumers,
family, stakeholders, and other Terros employees and Boys and Girls
Club employees. Actively leads group prevention or therapeutic
group activities at assigned Boys and Girls Clubs of the Valley
locations. *This position is funded by a grant, which is for a
pre-determined timeframe of 3 years. Benefits & Wellness

- Education/Experience:
- GED/High school diploma with a minimum of 1.5 years of
behavioral health experience or,
- An associate's degree in the field of medical or behavioral
health, or
- A bachelor's degree (not in the field of behavioral health) and
1 year of behavioral health experience, or
- A bachelor's degree in the field of behavioral, or
- A master's degree or higher in the field of behavioral
health
- Preferred - Bilingual proficiency in English and Spanish.
- Computer Skills: Microsoft Word, Excel, Outlook, electronic
medical record, NextGen preferred
- Valid AZ Driver License. Must have valid Arizona driver's
license, be 21 years of age with minimum 4 years driving
experience, and meet requirements of Terros Health's driving
policy
- Must have a valid Arizona Fingerprint Clearance card (Level 1)
or apply for an Arizona Fingerprint Clearance card within 7 working
days of employment
- Interpersonal skills which allow for professional and positive
relationships with co-workers, members, families, and other
community members
- Good communication skills, evidenced by a non-judgmental
approach, effective listening and positive interactions
- Basic understanding of the DSM-V preferred
- Certified Case Manager strongly preferred
- Must have or be able to attain CPR/First Aid certification
within 60 days of assuming role
- Must pass background check, TB test and other pre-employment
